In principle, when a woman is first registered a PGH - Pregnancy History form is administered to gather information on all her previous pregnancies. At subsequent visits any new pregnancies are recorded on a PON - Pregnancy Outcome Notification form. In practice the mechanism for deciding if a woman has completed a PGH form is imperfect and has been flawed, so that many women have in fact completed 2+ PGH forms.
