AMETHIST: As part of the AMETHIST consortium which in Zimbabwe is conducting a randomised controlled trial of the effectiveness of adapted microplanning and self-help groups on transmissible HIV (AMETHIST), we aim to adapt and transfer their model to the very different setting of South Africa and where there is no current sex worker programme. Study aim: Building on the work conducted in Zimbabwe, we will investigate how readily AMETHIST can be translated to our rural KwaZulu-Natal, South African settings with less established clinical services for FSWs and to understand the predicted impact on the HIV epidemics and cost-effectiveness. Due to no active sex worker programme, we first had to identify female sex workers through mapping exercises. This was to collect and synthesise data on the geographical spread and social organisation of sex work with an aim to subsequently conduct a respondent-driven sampling (RDS) bio-behavioural survey.

We conducted rapid participatory mapping in Phase 1 of the study - to understand the context of sex work, i.e. where, when and how sex work takes place. We then moved to Phase 2 to implement a respondent driven sampling (RDS) study consisting of 200 female sex workers (FSW) aged 18 and older living and working in the AHRI surveillance area - to understand demographic-behavioural characteristics, HIV prevalence among FSW and risk factors of HIV infection in this population.
Over a period of 7 months after mapping and identifying seeds, we collected demographic, behavioural and clinical data on FSWs with an interviewer-administered REDCAP survey. We also offered point of care HIV and syphilis testing, collect dried-blood spots (DBS) for HIV testing and viral load and vaginal self-taken swabs or urine for Chlamydia trachomatis CT/Neisseria gonorrhoeae NG/trichomonas vaginalis TV/ testing). Referral for treatment, ART for those who are HIV positive and PrEP for those who are HIV negative, will be provided through referral to existing adolescent and youth-friendly clinical research study services (Isisekelo sempilo clinics).
