MONARCH was a stepped-wedge trial of continuous quality improvement (CQI) conducted at the 7 clinics part of the AHRI original demographic surveillance area: Mtubatuba, KwaMsane, Somkhele, Gunjaneni, Machibini, Esiyembeni and Mpukunyoni clinics. Primary endpoint data were collected from the 7 participating clinics + the local district hospital as most deliveries occurred at the hospital, although no CQI activities occurred at the hospital.

Routine clinical data extracted from paper medical records
Antenatal visit. Defined by date.
NB: visit 11 is the delivery date. Up to 10 ANC visits were included in the analysis. If confv is missing or 0 that means the participant did not attend an ANC visit at that time (therefore, date and other information for that visit should be missing).
All ANC visit dates were entered sequentially based on actual attendance, regardless of time elapsed since last visit or when it was scheduled. E.g., if confv for visit 4 was missing, there should not be any data for visit 5 or beyond. There will be a date for visit 11 as that variable was created based on the delivery date.
